Baby squirming and fussing. What could be the problem?

Sounds like colic. My son had it and went to babiesrus and got baby gasX and i know you don't want to use unnecessary medicines but this stuff is way better than mylicone drops and you don't want him to be miserable. Ask your docor about it too because my son also had acid reflux and had to get on medicine. And yes they can have it even if they dont spit up because if my son spits up is abnormal lol
